We would like to know how we can improve our service to you and how you see our surgery and staff and would welcome your input.
To help us with this, we are setting up a virtual patient representation group so that you can have your say.
We will ask the members of this representative group some questions from time to time, such as what you think about our opening times or the quality of the care or service you received. We will contact you via email and will keep the surveys as short as possible so it shouldn’t take too much of your time.
We need young people, workers, retirees, people with long term conditions and people from non-British ethnic groups.
If you would like to take part, please fill in the form below and we will e-mail our survey to you.
We would like to make sure that we receive feedback from a representative sample of our registered patients, so in the box marked "your details" please state your gender, age and ethnicity.
We would also like to meet members face to face, so if you feel that you would like to come along to a meeting please
feel free to do so. Details of the next meeting are on the left of this notice. Alternatively, you can register your interest by filling in your details in the box below and we will contact you with further information.
Whether you take part in our virtual or reality group, your input will be most welcome and we hope that it will make you feel part of the practice.
